Psychiatric Nurse Practitioner, Outpatient Group Practice
Longstanding Illinois Behavioral Health Group that provides state of the art care to people suffering from mental health issues, psychiatric conditions and individuals who just want to grow and become healthier in their approach to life is seeking an independent, self-motivated Psychiatric Advanced Practice Nurse for outpatient behavioral health services.
Responsibilities:
- Perform complete initial psychiatric evaluation on patient to determine diagnosis and develop plan of care including medication prescription, if indicated.
- Revise, prescribe, and implement therapeutic regimen using an interdisciplinary approach.
- Monitor patient progress and provide collaborative treatment and communication with the treatment team.
- Provide timely documentation of each encounter with a patient.
- Ensure patient/family education regarding clinical condition and relevant aspects of care.
- Work in collaboration with treatment team members to ensure quality of care and consideration of services.
