The Primary Care Physician (PCP) in our organization demonstrates:
- Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients.
- Coaching for health: The PCP acts as a health coach, helping patients set health goals and following up on them.
- Simplifying for action: The PCP simplifies and prioritizes appropriately to help patients achieve their health goals.
We are an outcomes-focused, value-based organization, and for their panel of patients, the following metrics are regularly measured to help PCPs become successful and reach partnership status: patient admissions/thousand, using between 18-21 appointment slots per day, CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams. Culture is very important in the medical centers, and because PCPs are leaders in our organization, they are expected to help champion a positive culture of love, accountability, and passion along with center leadership.
The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings, and other meetings as required that promote patient health and company goals.
The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
- The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
- The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed.
- The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination for patients with acute and complex chronic health needs.
- The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
- For patients that are unable to come to the office, the PCP will engage with the transitional care team and other resources as available.
- PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons.
- Performs other duties as assigned and modified at manager’s discretion.

- Influence — PCPs must competently influence their patients and teams.
- Self-Care — Physician wellness is important for sustainability.
- Proficient in Microsoft Office Suite products and other relevant software.
- Ability and willingness to travel locally and occasionally regionally and nationally.
- Fluency in English, verbal and written.
- This job requires use and exercise of independent judgment.
EDUCATION AND EXPERIENCE CRITERIA:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
- Must be able to obtain a State Medical License for the state(s) in which he/she will be working.
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred.
- Must have a current DEA number for schedule II-V controlled substances.
- Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days of employment.
