The Population Health Care Manager will develop the Population Health Program, including managing care coordinators and monitoring quality outcomes affecting various patient populations. The Population Health Care Manager will work to develop this position and department to best serve the needs of the patient populations served.
REQUIRED:
- Bachelor of Science Degree in Nursing and a minimum of 3 years of experience in a clinical setting.
- Demonstrated leadership experience ability in a healthcare setting.
- Proficient computer skills, including Microsoft Office (specifically Word, Excel, and PowerPoint).
- Excellent oral and written communication skills.
- Good organizational, analytical, and problem-solving skills.
- Ability to function effectively with a wide variety of disciplines, including physicians and nurses.
- Confidence to communicate and collaborate with other community health care organizations and personnel.
- Able to participate with and/or lead a team to accomplish an assigned project.
PREFERRED:
- Certification in CMQ, CPHQ, Population Care Coordination, or other quality improvement methodology.
- Master's degree in nursing or health related field.
- Previous experience in a Primary Care Setting.
- Previous experience in case management, care coordination, or patient navigation.
