Overview
Plans and manages patient care according to each patient's needs. Interviews patients and records their medical history and physical condition. Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration. Provides routine care for patients.
Responsibilities
- Provide 1:1 care for client always following client’s plan of care while maintaining client's dignity and independence.
- Monitor clients’ vital statistics and report abnormal findings to the case manager and physician assessments.
- Perform medication administration, feedings, oxygen, ostomy care as needed per client's plan of care and provide assessments of results.
- Complete routines as established by other care professionals such as Physical Therapist, Occupational Therapist, and Speech Therapist.
- Ensure infection control policies are maintained at all times, report possible communicable diseases or infections, and ensure the overall implementation and practice of infection control procedures.
- Accurately document nursing actions of all care given and communicate with family, case manager, and physicians; document the progress and outcomes for established goals and inform physician, case manager, and families of changes in client's medical condition and needs.
- Order supplies and medications, as needed, to ensure adequate amounts are on hand at all times.
- Ensure all patient needs are being met in accordance with the instructions of the physician.
- Monitor incidents involving individuals; ensure incident reports are completed timely and accurately, and case managers are informed; examine the individual following the incident.
- Ensure all medical equipment is maintained and report faulty equipment to Durable Medical Equipment for repair.
- Other duties as assigned.
Qualifications
- Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
- CPR Certified.
- Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
- In-depth understanding of patient care policies and regulations.
- Familiarity with symptoms and signs of improvement for several medical conditions.
- Moderate amount of travel between facilities may be required.
