The Grand Rehabilitation and Nursing Center at Guilderland, located in Altamont, NY, is seeking LPNs (Licensed Practical Nurses).
- Position can be full-time or part-time
- Seeking LPNs for all shifts (day, evenings, or overnights)
- $4,000.00 sign-on bonus for full-time! ($2,000.00 for part-time)
- Rates are $29.00 to $37.00 per hour
- Benefits include PTO, Health Insurance, 401k with partial match

Responsibilities & Requirements:
- Utilize the nursing process (assessment, planning, implementation, and evaluation) for the best care of our residents
- Provide health and wellness checks to our residents on a regular basis
- Assist in the development and update of individual service plans for each resident
- Maintain ongoing communication with residents, their families, and their physicians regarding their needs
- NY State license must be in good standing
