Kaiser Permanente/ The Southeast Permanente Medical Group (TSPMG)
The physical therapist in this role will be responsible for the physical therapy management of low back pain patients under the direction and supervision of the Chief of Pain Management. The physical therapist will also assist the Quality Resource Management (QRM) team with referral review of external PT referrals related to back pain. This work will be under the supervision of the Physician Director of QRM.
Major Responsibilities/Essential Functions
- Evaluates patients along with Nurse Practitioner and interprets evaluation findings to determine the nature and extent of dysfunction. Establishes treatment goals and plans treatment to achieve established goals with Nurse Practitioner.
- Initiates discharge planning from physical therapy. Implements the treatment program through direct treatment of the patient, may delegate a portion of the treatment plan to appropriate supportive personnel, and coordination of the treatment plan with other health care personnel as appropriate. Provides advice and education to patient and family. Regularly assesses clinical signs and symptoms.
- Discharges patient from physical therapy when the patient has received maximum benefit from physical therapy. Provides accurate, timely documentation of physical therapy patient management. Demonstrates progress toward meeting the criteria for professional practice.
- Review of physical therapy referrals for back pain. A portion of FTE will be designated for referral review and utilization management of external physical therapy resources.
Minimum Education
- Graduate of an accredited physical therapy curriculum with a bachelor’s or master’s or doctoral degree with certification in physical therapy or foreign-trained equivalent by date of hire.
Minimum Work Experience and Qualifications
- Working knowledge of all federal, state, local and regulatory requirements, including Medicare/CMS.
- Two (2) years of post-training nurse practitioner experience in primary care.
Preferred Work Experience and Qualifications
- Prior multi-specialty medical group experience.
- Preferred/priority consideration for experience at Kaiser Permanente/TSPMG in a clinical field.
- Strong Health Connect/Epic Skills.
- Previous experience in pain management/orthopedic surgery or other related departments.
Required Licensure, Certification, Registration or Designation
- Physical Therapist License (Georgia) required at hire.
- National Provider Identifier required at hire.
- Georgia Physician Assistant license received from the Georgia Composite Medical Board.
