Coverage area: Oversight of Physical therapists in Sacramento, Yolo, Solano, and Napa Counties.
What You’ll Do:
The Clinical Supervisor - Therapy is responsible for ensuring that patient care is coordinated and managed appropriately. They serve as the first point of contact for field clinicians to ensure support is easily accessible in a timely fashion. This is a leadership position responsible for the oversight of clinicians providing patient care in their place of residence.
This is a hybrid position involving fieldwork and onsite responsibilities at the 21HHS office:
2 Days in the Sacramento Office
3 days in the field: 12 patient visits each week (supervisory visits of field clinicians)
What you'll do as the Clinical Supervisor - Physical Therapy:
- Responsible for in-field oversight of clinicians who provide care and treatment to patients in their residences.
- Handles incoming calls timely and follows up as necessary to ensure appropriate actions are taken to meet customer needs.
- Coordinates training and educational programs for nurses within the field to improve clinical outcomes and competencies.
- Assesses staff learning needs and formulates specific plans and goals for the next evaluation period with employees, performing administrative call duties, and serving as a role model and resource for staff.
- Functions as the first point of contact for field clinicians to assist in patient care escalations.
- Coordinates at least bi-weekly Case Conferences with the clinical team to discuss patients' progress and revise the care plan as needed to meet planned outcomes.
- Reviews and evaluates each case by reviewing services provided by clinicians, conducting conferences, record reviews, discussing and verifying impressions, and instructing and guiding clinicians to promote effective performance and quality home care services.
- Reviews patient clinical diagnoses, medications, procedures, and clinical courses.
- Assists clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing a plan of care.
To Be a Success:
- Two years of management or supervisory experience in a rehabilitation therapy department or program, preferably home care.
- Possesses a degree from a baccalaureate or master's program in an applicable discipline approved by an accredited organization.
- Current CA Physical Therapist license.
- Current Basic Life Support (BLS) certification.
- Valid CA Driver's License with an acceptable driving record.
- All licenses and certifications must be current at the time of hire and sustained throughout employment.
Salary: $143,000 - $163,000 a year, plus a generous bonus paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits, and a 401k (up to 4% match).
