Responsibilities:
- Conduct assessments, medication evaluations and management, treatment planning, and discharge planning services to adult individuals and families.
- Coordinate treatment plans, referent relations, payor relations, and intervene with client support systems.
- Administer care that is individualized and consistent with wellness center standards.
- Provide basic care management services to mentally ill patients in a clinical setting.
- Communicate with follow-on providers after patient discharge from the program.
- Assist patients and families in addressing concrete service needs (vocational, education, entitlement).
- Conduct intakes and assess admission criteria for the wellness program.
- Provide therapeutic interventions, medication assessment, and crisis response to new and existing clients.
- Offer psychotherapy services and participate in diagnosis, treatment planning, rehabilitation, evaluation, and follow-up of patients.
- Request personality, intelligence, and other clinical tests and guide therapy based on results.
- Maintain documentation of services, including treatment plans, progress notes, and compliance with wellness center and third-party agency requirements.
- Provide crisis assessments for walk-in patients experiencing emotional distress and initiate care regimes.

- Participate with the healthcare team to maintain continuity of care.
- Consult with and advise representatives of interested groups regarding patient care and therapy.
- Provide consultation and health education to staff relevant to treatment and medication needs of consumers.
Education:
Completion of a master’s degree or post-master’s degree in Nursing within the psychiatry specialty area from an accredited college or university.
Professional Certification and License:
Possession of a current license to practice as a Psychiatric Nurse Practitioner as granted by New York State.
Specialized Knowledge:
- Knowledge of principles of psychotherapy applicable to care, treatment, and rehabilitation of children and adults with behavioral, developmental, or psychological disturbances.
- Knowledge of psychological testing devices and methods.
Other Characteristics:
- Knowledge of record-keeping and report writing techniques.
- Ability to organize and carry out new programs and procedures.
- Current Basic Life Support (BLS) certification.
- Complies with all OMH and Justice Center requirements.
