Job Summary:
The Providence Center Case Manager ACT Team will coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric symptoms.
Duties and Responsibilities:
- Coordinate a variety of services.
- Provide supportive counseling, problem solving, and limit setting.
- Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with people who have severe persistent mental illness and/or substance abuse issues, as it relates to case management.
- Monitor client status during spend down process, complete necessary DHS applications.
- In cooperation with Managers or Emergency Services Staff, assist with assessments, crisis interventions and risk management as needed.
- Negotiate and advocate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
- Transport consumers in own vehicle to planned activities and appointments.
- Complete recordkeeping in accordance with Health Information Services (HIS) and other TPC requirements within required time frames and abide by TPC confidentiality policy.
- Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
- Attend mandatory in-service trainings and other trainings required.
- Attend case presentations and conferences.
Requirements:
- Associate's degree required. Bachelor's degree from accredited four-year College or University in Human Services or related field preferred.
- Experience in Human Services field preferred.
- Must have valid driver's license and insured auto.
- Must be able to work flexible hours.
