Nurse Practitioner

Our team of highly skilled wound care specialists provide advanced treatments in skilled nursing facilities, which dramatically cut down on hospital readmissions. Our leadership has over 25 years of experience in the skilled nursing space, and we’ve applied our extensive industry knowledge to our proprietary EMR system, in-facility treatment modalities, and insurance-compliant documentation.
Duties and Responsibility
- Assessment and Treatment: Conduct thorough assessments of patient wounds, which may be chronic or acute, and develop individualized treatment plans.
- Implement advanced wound care techniques, including debridement, dressing selection, skin grafts, and infection control measures.
- Collaboration with Healthcare Teams: Work closely with other healthcare professionals at each facility to coordinate care and provide updates on patient progress to ensure continuity of care.
- Manage a travel schedule to visit multiple skilled nursing facilities on a regular basis.
- Ensure timely and efficient care delivery.
- Maintain accurate and comprehensive patient records, documenting all assessments, treatments, and patient/family interactions.
- Participate in quality improvement initiatives, staying updated with the latest advances in wound care and applying best practices in treatment.
- Complete documentation accurately and in a timely manner.
- Educate patients and their families on wound care, prevention, and management.
- Monitor patient progress and adjust treatment plans accordingly.
Requirements:
- Valid License as a NP.
- Minimum of 1 year’s experience in advanced wound care is highly preferred.
- Certification in wound care (e.g., CWOCN, CWS, WCC, WOCN) is a plus.
- Ability to travel to multiple locations.
- Strong interpersonal skills and the ability to work independently.
- Excellent organizational and time management skills.
- Wound Expert EMR knowledge preferred.
Physical/Mental Demands and Work Environment:
- Frequent standing, walking, and sitting required for extended periods.
- Requires visual and hearing acuity.
- Able to lift a minimum of 25 pounds.
