Acenda Integrated Health is recruiting for a Client Advocate to support the Assertive Community Treatment Program in Gloucester, New Jersey.

Program Overview:
The Program for Assertive Community Treatment (PACT) is an intensive, team-based behavioral health service that offers individualized support in the community to people with serious mental health conditions and high service needs. PACT assists individuals in their recovery from mental health conditions and in developing fulfilling lives, as well as increasing their time and engagement in community settings rather than institutions such as hospitals and jails.
Position Summary:
- Provides face-to-face individualized clinical support services for consumers who need consistent contact to ensure the highest level of optimal psychosocial functioning.
- Operates as a strong community liaison to ensure that individuals can navigate and understand all services that are available.
- Collaborates with Acenda and Community Providers to link individuals into services needed.
Requirements:
- A minimum requirement is a High School diploma and two years of experience working with individuals with serious and persistent mental illness.
- A Bachelor’s degree in psychiatric rehabilitation and two years of experience in the provision of mental health services preferred.
- Must have and maintain a valid driver's license with an acceptable driving record.
