While under the supervision of the Health Homes Project Director, the Nurse Care Manager oversees the coordination of healthcare services for persons served and provides limited supervision/guidance to Wellness Coaches.
Essential Job Functions:
- Provide all services and functions within the system's Code of Conduct; adhere to all company policies for employment.
- Demonstrate understanding of health service systems and assist program participants in identifying, securing and sustaining a range of resources needed to facilitate independent healthy living.
- Serve as a member of a multidisciplinary team, supporting the development of various wellness plans based on individualized strengths, needs, abilities, and preferences of the person served; ensures implementation of wellness plans with assistance and support from team members and community resource providers.
- Process referrals for services, gather all pertinent physical and mental health information, and provide health recommendations as appropriate.
- Organize initial appointments and conduct health screenings; can conduct regular face-to-face meetings with participants to discuss wellness goals and conduct physical assessments/treatment.
- Contribute to the Comprehensive Care Plan (CCP) in partnership with participants' team members and aides in the completion of healthcare goals.
- Develop member registries.
- Coordinates participants' care with external community providers.
- Records and documents participant’s health information into the organization's electronic medical record system.
- Advises and supports Wellness Coaches to ensure quality coaching services; reviews wellness interventions as necessary and provides feedback.
- Evaluate services that are provided to ensure that necessary improvements are made to increase outcomes.
- Assist program participants with learning skills for successful personal health management and self-advocacy.
- Monitor and assess program participants' functioning and progress; update team members with information relevant in supporting the physical wellness plan goals.
- Engages participants in the wellness process by fostering natural supports, providing health education, monitoring development of healthy-living skills, and coaching on wellness plan action steps.
- Participate in organizational processes, as directed by the Project Director, which may include quality assurance, peer record review, and health/safety activities.
Knowledge, Skills, and Abilities:
- Thorough knowledge of principles, theories, and practices of nursing in behavioral health and general health care.
- Knowledge of the policies, procedures, and regulations of PFH programs to which the associate is assigned.
- Detailed knowledge of information and techniques needed to rehabilitate physical and mental ailments and provide alternative treatments.
- Skilled in effective entering, transcribing, recording, and storing information in written or electronic form.
- Aptitudes to develop, coordinate, and evaluate services within assigned program responsibilities.
- Effectively apply techniques of a complex nature to make a decision or solve a problem by using logic to identify key facts, explore alternatives, and propose quality solutions.
- Capability to communicate ideas clearly and effectively to produce clear and concise reports.
- Some knowledge of supervisory and training techniques.
Experience and Education Qualifications:
- Licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) is required.
- Certification in wellness-based programs is preferred; established experience and participation in wellness coaching/domains is required.
Supervisory Requirements:
- Provides guidance and limited supervision to Wellness Coaches in coordination with the Director.
- Communicate the strategic direction of the organization and encourage participation by all team members.
- Provide leadership and guidance to all aspects of the department.
- Take an active role in monitoring the identification, development, and execution of strategic objectives.
- Involve, as appropriate, all team members to achieve goals.
- Effectively communicate to team members any changes and newsworthy events within the department or company.
- Handle difficult team member situations directly, using appropriate discretion and Human Resource advice to show respect for the individual.
- Champion change and effectively manage the implementation of new ideas.
- Reinforces team approach throughout functions; support and solicit input from team members at all levels within the company.
Employment Requirements:
- Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.
- Completion of New Hire Orientation at the beginning of employment.
- All training requirements including Relias at the beginning of employment and annually thereafter.
- Current driver’s license, acceptable driving record and current auto insurance.
Physical Requirements:
ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up to 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift. Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
