Job Title: Case Manager
Division/Program: Transition Age Youth Program - Alameda
Starting Compensation: 22.36 - 26.83 USD Per Hour
Working Location: San Leandro: San Leandro Center (In-person position)
Working Hours/Shift: Monday-Friday, AM Shift

What you bring to SBHG (qualifications):
- 4 years of experience in a social service field OR
- Associate Degree plus 2 years of experience in a social service field OR
- Bachelor’s degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service
- Valid California Driver's License
How you will make a difference (job overview):
This position has primary responsibility for case management services to clients and their families referred through Stars’ Transition Age Youth (TAY) Program.
Division/Program Overview:
- Outpatient, community-based program that serves youth age 18-25 with serious and persistent mental health issues
- Program provides intensive case management, outreach and engagement, medication support, as well as, mental health counseling, peer groups, and supportive services to assist TAY transition to adult life.
- Services offered are trauma informed and culturally responsive
