ProHealth Care Medical Associates is seeking an experienced and collaborative Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Family Medicine team at the Watertown Clinic. This position offers the opportunity to be part of a comprehensive care team, delivering high-quality, patient-centered care across all ages in a collaborative care model with our Family Medicine physicians.
Key Responsibilities:
- Provide advanced practice services to patients of all ages in a primary care setting under a collaborative agreement with a designated PHCMA physician.
- Perform patient assessments, including health data collection, diagnosis, treatment, and referrals when necessary.
- Manage a panel of patients in collaboration with a physician, providing consistent care throughout the patient’s life cycle.
- Collaborate with other healthcare providers and participate in on-call rotations for the clinic.
- Develop and implement individualized care plans in partnership with patients, families, and other health care providers.
- Coordinate care across the continuum, including discharge planning and community resources when appropriate.
- Ensure compliance with PHCMA standards, State of Wisconsin regulations, and the PHCMA Scope of Practice and Collaborative Agreement for Nurse Practitioners.
- Participate in performance improvement programs and contribute to the growth and development of the department.
- Provide mentorship and education to staff and students, promoting professional development.

Schedule Details: Monday thru Friday - 35 patient contact hours
Qualifications:
- Current certification as a Nurse Practitioner (NP) or Physician Assistant (PA) with licensure in the state of Wisconsin.
- Experience in Family Medicine or Primary Care preferred.
- Ability to work independently while collaborating effectively with a multidisciplinary team.
- Strong clinical knowledge and competence in assessment, diagnosis, and treatment.
- Commitment to providing compassionate, high-quality care to patients across the lifespan.
Licensure:
- Current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating NLC state.
- Certification as an Advanced Practice Nurse Prescriber by the Wisconsin State Board of Nursing.
- Certification by ANCC or AANP for Nurse Practitioners or Physician Assistant license issued by the state of practice.
- PA-C certification by the NCCPA.
- DEA registration or eligibility to obtain.
Certifications:
- AHA Basic Life Support for the Healthcare Provider (BLS) or an equivalent certification.
Education:
- Master’s degree in Nursing for Nurse Practitioners.
Experience:
- A minimum of 2 years of clinical experience is preferred; new graduates are considered if highly qualified.
