Eliot’s Behavioral Health Division provides an integrated and comprehensive network of services to adults with psychiatric disabilities that are evidence-based, providing rehabilitation and hope to those in services. Eliot's Behavioral Health Community Partner (BHCP) program is an innovative Care Management approach for MassHealth members with complex medical, behavioral health, and social needs who are part of the new Accountable Care Organization (ACO) redesign within Massachusetts.
Eliot is seeking a Care Coordinator to provide primary coordination of service delivery specific to the needs and preferences of its BHCP enrollees. The Care Coordinator will work with a multidisciplinary team to provide person-centered Comprehensive Assessments and Treatment Planning. In this role, you will develop relationships with enrollees, their PCP, and Care Team. Eliot is seeking a highly motivated and organized individual to manage a caseload of enrollees, provide independent care management, and actively connect to all services and community supports available to each enrollee.
Responsibilities:
- Promote hope and empowerment through meetings and support with individuals.
- Empower individuals to exercise autonomy in all aspects of their life. Assist enrollees with identifying choices and developing skills in self-advocacy.
- Provide side-by-side interventions to enrollees that focus on problem solving, skills training, modeling behavior, optimism, and encouragement.
- Assist the team and enrollees in providing ongoing assessment of symptoms and responses to Care Team interventions.
- Participate in the development of Assessments and Treatment Plans for each engaged enrollee.
- Ensure that treatment goals are culturally competent, strength-based, realistic, attainable, and promote recovery focused on an enrollee’s goals for wellness.
- Engage each enrollee in their treatment planning process, ensuring their preferences are embraced, collaborated with the enrollee’s PCP, and shared with the enrollee’s Care Team.
- Maintain all documentation related to assessments, treatment plans, enrollee contacts, collateral contacts, and any other necessary documentation.
- Community-based position.
Schedule: Monday-Friday, 9am-5pm
Salary: $50,000 annually
Bilingual salary: $53,000 annually
Location: Community-based position that serves the Acton MA area; Please note: this position is based out of our office in Saugus MA.
Bilingual candidates preferred and eligible to receive a 6% differential if fluent in a needed language.

Education: Preferred - Bachelors
Experience: Preferred - Working with adults with serious and persistent mental illnesses
