The Diabetes Care and Education Specialist for Maine Medical Center (MMC) provides direct patient education for persons with diabetes and their families to prepare them for safe diabetes self-care after discharge. This consulting service receives referrals from providers and nurses throughout MMC. Additional responsibilities include acting on outcomes data, assisting with clinical diabetes management protocols or pathway development, educating healthcare professionals, and acting as a healthcare resource for MMC and MaineHealth. The Diabetes Nurse Care and Education Specialist will collaborate his/her work efforts with the inpatient units and associated care providers as well as the multidisciplinary inpatient glucose control project management team.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: Knowledge of nursing theory, practice and education sufficient to coordinate care for diabetic patients on an inpatient basis, acquired through the completion of a BSN preferred, must matriculate within one year with completion attained within five years Bachelor's Degree program in nursing, plus two years progressively more responsible related experience in the area of diabetes education.
- License/Certifications: Current license and registration to practice as a Registered Nurse in the State of Maine. Currently certified as a diabetes educator.
- Experience: See "Education"
- Ability to function independently in an expanded role as well as participating effectively within the team.
- Interpersonal skills sufficient to counsel and instruct patients and families; to serve as a resource person for other health care professionals in the institution who are involved in the care of diabetic patients, and to conduct formal educational programs.
- Analytical and organizational abilities sufficient to serve as a resource, to plan and help implement the clinical program, and to promote the development of family-centered care.
