OverviewCHI Saint Joseph Medical Group, now part of CommonSpirit Health formed between Catholic Health Initiatives (CHI) and Dignity Health, includes 275 providers delivering quality care to patients across 88 locations in central and eastern Kentucky. Our full-service network delivers a variety of Primary Care Services including Family, Internal, Geriatric, and Pediatric Medicine in addition to several specialties to deliver custom care based on the unique needs of our patients.
ResponsibilitiesESSENTIAL KEY JOB RESPONSIBILITIES:
- Demonstrates the knowledge, skill, and coordination to provide nursing care and guidance to the OBGYN patient from first contact and throughout follow-up, serving as an essential link between patients and all other care providers.
- Systematically and continuously performs the functions of assessing, planning, implementing, and evaluating the care according to ACOG and CHI Saint Joseph Health standards of practice.
- Facilitates the coordination of patient care services to assure excellence in patient care and patient flow by: A) Following the patient through the care continuum/experience, eliminating operational (i.e. scheduling, test results, etc.) barriers to service; B) Working closely with other health care disciplines to ensure timely appointments, results reporting, financial need referrals, communication, patient care, and follow-up.
- Interfaces with other healthcare teams for appropriate referrals/services such as the social worker, pastoral care, dietician, community services, financial counselor, pharmacy coordination, home care, smoking cessation, etc.
- Initiates and documents patient teaching including family and significant others based on assessment of needs, including responding to patient requests for information regarding the disease process, expected side effects of treatment, and community resources. Documents the above appropriately as well as communicating teaching completed to the patient's other healthcare providers as appropriate.
- Tracks and communicates monthly referrals by disease site and distribution as requested.
- Continually assesses the patient's progress and educational needs, making adjustments to care based on the assessment; consults with primary physician or designee and other members of the care delivery team as needed; refers to appropriate physicians, NPs, or programs when needed.
- Serves as a resource for community educational events such as health fairs, screenings, support groups and utilizes the resources of CHI Saint Joseph Health organization.
- Coordinates pertinent patient information, notifying appropriate MDS.
- For gynecologic patients, the RN will create and develop long-term relationships. The RN will monitor high-risk patients’ pap/colpo results and ensure follow-up. The RN will ensure patients receive pre and post-op education and follow-up from ED visits, mammograms, OR pathologies, and abnormal HCGs.
- For obstetrics patients, the RN will create the most welcoming OB experience and develop long-term relationships with these patients. The RN will perform the initial patient intake, provide new OB education, ensure OB visits are scheduled out for the duration of the pregnancy, ensure that proper testing per gestational age is completed, follow up after ED/L&D visits, and follow up after delivery. The RN will also ensure high-risk patients are seen for their postpartum visits, follow up postpartum, and follow up on GTT results.
- The navigator position is to ensure that patients are being seen per protocol and ACOG guidelines.
- Other duties as assigned by management.
Qualifications- Required Education: Required Education for Staff Job Levels ADN or Prefer Bachelors of Science in Nursing
- Required Licensure and Certifications: KY RN licensure
- Required Minimum Knowledge, Skills and Abilities: Minimum of 5 years of experience in inpatient or outpatient OBGYN setting, or related experience required
- PREFERRED Qualifications: Experience/knowledge in community health setting or services
