We are looking for a Registered Nurse (RN) to join our care team. In collaboration with other members of the healthcare team, the Registered Nurse is responsible for organizing, coordinating, and providing care coordination and care management services to patients. In this unique role, the RN will work with collaborating providers and Patient Navigators to determine patient escalations for urgent needs, ensure provider chart preparations are completed prior to patient meetings, and ensure our patients have a “Wow” care experience. This role will work out of our headquarters in Columbus and work across the care team, helping us build new practices and protocols.
What you’ll do in the role:
- Serve as the first line of clinical expertise for all inbound clinical questions and patient needs, ensuring the providers are covering the most critical patient inbound questions and requests.
- Create clinical pearls of standardization for the most commonly asked clinical questions.
- Incorporate data tracking for care gap closure, and streamline a methodology to manage and report on care gap needs.
- Interface with patients when a care gap has been identified to ensure connection to appropriate care and manage follow-through.
- Become an expert in patient care processes to improve upon clinical workflows.
- Document all patient-related activity in the appropriate EHR, per policy.
- Ensure patient charts are complete and appropriately represent the status of a patient, preparing providers for the highest quality patient visits.
- Participate in huddles with other members of the care team to help ensure patient’s needs are met, and the developed processes and workflows are effective.
Education & Licensure Requirements:
- Registered Nursing degree required.
- 3+ years of relevant experience preferred.
- Clinical knowledge to understand and evaluate patient information and case management.
- Use of MS office applications, EHRs, and other reporting databases strongly preferred.
Other Skills or Qualifications:
- Experience working with electronic medical records required.
- Ability to work collaboratively with people of diverse cultures and lifestyles.
- Ability to communicate effectively with providers and medical staff.
- Excellent organizational skills and ability to handle multiple priorities while remaining calm and professional.
- Ability to be self-motivating and work independently.
- Use of MS office applications, EHRs, and other reporting databases strongly preferred.
