Qualifications:
- Licensed as a family Nurse Practitioner in the state of Utah
- FNP degree from an accredited school or University
- Dedicated to patient centered care
- Excellent interpersonal, organizational and communication skills
- Ability to create a positive work environment
- Ability to identify and solve problems
- Friendly and capable of respectfully communicating to both clients and staff.
- Self-starter
Responsibilities:
- Conduct physical assessments.
- Perform and interpret diagnostic tests.
- Diagnose, treat, and evaluate chronic illnesses.
- Evaluate patient progress and treat patients appropriately.
- Provide screenings, interventions, counseling, and prescribe medications as needed to patients.
- Ensure patient information, visit history, referrals, lab follow-up, phone calls, and consults are carefully and completely documented.
