Job Summary
Responsible for development, coordination, and management of individualized service plan for individuals in Delaware Regional Office programs funded by DDDS. This position will provide service to the participant as well as manage overall service plan.
Primary Duties
- Ensure participant’s individualized plan of service (ISP) reflects relevant personal goals, includes activities to meet the goals, and is based on individual’s input. Invite any member of support network individual identifies to provide input. Progress toward goals is documented daily and plans are reviewed and revised at least twice a year, or as needs of participant change.
- Build and maintain professional working relationships with participants, families, residential providers, DDDS Case Managers, and other members of participant support team; ensuring communication is professional and timely, soliciting input and feedback. Participate in team meetings organized by participants support team members.
- Maintain participant files (digital, hard copy, databases) according to Service Source DE Policies and Procedures and meeting requirements of all regulatory entities.
- Provide direct service to participants; planning and implementing activities, providing one-on-one supports, supporting Community Integration Specialist with daily routines.
- Demonstrate knowledge and practice current DDDS Policies and standards that must be observed.
- Generate all required reports including monthly compilation of service notes and quarterly reports required by DDDS.
- Participate in intake meetings with Program Manager.
- Orient new participants informing them about daily routines, safety standards as well as facilities on site.
- Develop new activities as needed to meet emerging interests of participants.
- Represent Service Source DE in a professional manner.
- Identify Community resources, events, partners to ensure maximum community integration.
- Perform other responsibilities as assigned.
Qualifications: Education, Experience, and Certification(s)
Bachelor’s degree in Special Education, Psychology or related social service field and 3 – 5 years’ work experience preferred.
Valid driver’s license to perform work-related travel required. Eligible drivers must have a good motor vehicle record (MVR). Case Manager will have to drive Service Source vehicles for community activities during the program day.
Knowledge, Skills, and Abilities
- Prior work experience in social service field is preferred.
- Knowledge of developmental disabilities.
- Must be organized, self-motivated and able to work independently.
- Must be able to communicate effectively, in verbal and written form.
- Must be flexible.
- Proficiency in Microsoft skills or the ability to learn.
