Ambulatory Clinic RN Care Coordinator - Colorectal Oncology/Surgery
Position Summary / Career Interest:
The Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who is a clinical expert providing ethical, evidence-based, safe, and compassionate nursing care. They focus on patient safety and quality nursing care by identifying patient needs, performing clinic-related patient care, conducting health education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes.
The RN Care Coordinator provides support to assigned physicians by communicating with the patient/family/caregiver and relaying information to the physician. They also support and provide nursing care services during the physician's clinic sessions, ensuring the patient/family/caregiver understands the plan of care.
Responsibilities and Essential Job Functions:
- Responsible for management of patient flow, triage, and direct patient care.
- Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager.
- Manage the care of the patient population by developing expected patient outcomes and defining appropriate resource utilization.
- Provide nursing support/coordination to optimize patient outcomes and improve quality of life through clinical and educational programs.
- Support patient self-management of disease.
- Coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
- Manage high-risk patient care, including patients with multiple co-morbidities.
- Develop relationships with patients and assess their physical, psychosocial, financial, and environmental healthcare needs.
- Educate patients and ancillary personnel about healthcare needs and safety issues.
- Utilize electronic tools, such as EMR, to document assessments, clinical findings, and treatment plans.
- Observe, record, and report patient conditions and reactions to treatments.
- Maintain patient confidentiality and compliance with practice standards.

Required Education and Experience:
- High School Graduate
- Bachelor's Degree in Nursing
- 2 or more years of RN experience
Required Licensure and Certification:
- Basic Life Support Heartsaver (BLS HS) - American Heart Association (AHA) or obtained within 14 days.
- Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing.
Knowledge Requirements:
- Basic typing and word processing ability.
- Excellent analytical, critical thinking, and problem-solving skills.
