Creative Minds services adults with developmental disabilities who live independently in their own home receiving our Independent Living Skills (ILS) and Supported Living Services (SLS) programs. As a Case Manager, you will manage a caseload of 15+ clients. You will oversee a team of caregivers that you must lead, and you will work alongside other Case Managers, our Program Manager, and Program Director.
Our Case Managers coordinate and schedule client supports and services, mentor and manage a team of DSP staff, provide quality assurance, complete annual and semi-annual reports, intake assessments, and provide 24 hours emergency response at least one weekend per month.
Preferred Qualifications:
- Knowledge/experience with the Regional Center and DDS protocol
- Knowledge/experience with ILS and SLS programs
- Field experience
- Fingerprint/background clearance – no felonies or criminal record
- Acceptable DMV record
- Valid CA Driver’s License
- Working and reliable vehicle, auto insurance
- Health screening clearance
- Eligible to work in the U.S.
Rate of pay can be up to $24.00/hour based on experience and education. Workdays are Monday through Friday, with one weekend per month “On-Call”.

We provide our services in the San Fernando Valley, Santa Clarita, and Antelope Valley areas. It will be necessary to travel throughout these areas as part of the responsibilities associated with the position. A valid California Driver’s License, car insurance, and a reliable vehicle are requirements. A mileage allowance will be provided for travel.
