Core Responsibilities:
- Contribute to the advancement of the quality program within Oak Street Health, leading to improved health outcomes and admission prevention.
- Provide education to clinic team members to improve quality performance, including quality compliance, quality exclusions, quality documentation, and recommended strategies to improve quality scores.
- Create/maintain quality worklists for clinics to enable timely review of quality gaps and track progress.
- Analyze reports to identify trends and opportunities per clinic/care team.
- Review patient charts/records in EMR (electronic medical record) systems and upload documentation where applicable.
- Other duties as assigned.
