POSITION SUMMARY: The Community Behavioral Health program focuses on providing support to clients to overcome cultural and linguistic barriers in order to gain access to needed behavioral health services with particular focus on substance use disorder (SUD) and addictions.
ESSENTIAL JOB FUNCTIONS:
- Connect clients to appropriate clinical services, therapy and community resources while ensuring a holistic approach that addresses physical, mental, social, and spiritual well-being.
- Works closely as a member of a treatment team in collaboration and coordination of care while monitoring, documenting, or reporting of client progress towards goals.
- Identify client’s risks, strengths, needs, resources and abilities to address barriers to meeting client goals and accessing resources.
- Develop and maintain a strong collaborative relationship with a wide range of community partners, resource networks, and team members.
- Document all activities in a timely and accurate manner.
- Other duties as assigned.

QUALIFICATIONS:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
Education and/or Experience: At least one (1) year of experience working directly with individuals with substance use issues.
Credentials: Substance use credentials and/or combined experienced considered may include: CADC, CRM, QMHP, NCAC, LCSW, etc.
General Knowledge, Skills and Abilities: Ability to work well under pressure and work effectively in a multicultural setting, demonstrating excellent organizational skills and interpersonal skills. General experience with Google Suite and Microsoft Office. Ability to strictly adhere to HIPAA standards to ensure client confidentiality. Ability to develop strong collaborative relationships with a range of community partners.
Transportation: This position requires reliable transportation for business purposes, such as between work locations and community settings. This position is responsible for driving clients in a personal and/or agency vehicle.
Language Skills: Excellent communication skills with the ability to read, write, and speak in English. Multilingual and culturally diverse individuals are encouraged to apply.
Clearances: Standard LCSNW criminal clearance, including national sex offender, ORCHARDS, SAM/OIG, motor vehicle records, and valid driver’s license and auto insurance required.
