POSITION PURPOSE
As a Registered Nurse, you will provide vital oversight and nursing care to our participants, ensuring high-quality service delivery and support for their health needs.
WHAT YOU WILL DO

- Create a warm, welcoming environment for participants and team members.
- Collaborate with interdisciplinary teams to develop and implement care plans.
- Conduct assessments, manage treatments, and provide skilled nursing care.
- Educate participants and families on health topics and self-care interventions.
- Maintain accurate medical records and documentation.
- Participate in quality improvement initiatives.
- Support new participants in integrating into the PACE program.
- Provide on-call coverage and assist in various program activities as needed.
MINIMAL QUALIFICATIONS
- Graduate of an accredited Nursing program; BSN preferred.
- Current, unencumbered RN license in the state of practice.
- Active CPR certification required.
- Minimum of 2 years of experience with the geriatric population.
- Valid driver’s license and proof of insurance.
- Strong clinical skills, attention to detail, and problem-solving abilities.
- Excellent communication skills and proficiency in Microsoft Office.
