Family Nurse Practitioner Needed in Ohio – Less than 1 hour to Columbus!!
Health Center is seeking a Nurse Practitioner to work in our Medication Assisted Treatment program using Vivitrol for treating opiate dependence.
Vivitrol doesn't require any special DEA notation and it's a non-opiate. They do not do any Suboxone, so no worries there.
JOB SUMMARY
The Family Nurse Practitioner, in collaboration with Health Center physician(s) may provide preventive and primary healthcare services. Care provided will be consistent with the nurse's education and certification and in accordance with the rules adapted by the Ohio Board of Nursing and section 4723.43 of the Ohio Nursing Revised Code.
SPECIFIC RESPONSIBILITIES
- Develops and maintains standards of care in accordance with the Ohio Nursing Revised Code.
- According to standards of care and protocols, provides care to patients within guidelines of specialty education. Provides preventive and primary care services, evaluates and promotes patient wellness.
- May make referrals to the collaborating physician or specialty physician as indicated.
- Participates in the development and implementation of the Health Care Plan; the Quality Improvement Process; health education protocol, and community education and outreach programs.
- May participate in the supervision of ancillary staff and students.
- Is required to receive annual OSHA training regarding blood borne pathogens and infectious waste; and attend the monthly Clinician Staff Meeting unless excused by the Chief Medical Officer or Chief Executive Officer.
- Participates in team meetings to discuss selected patients on each day's schedule with follow-up tasks (often referred to as "Team Huddles").
- As part of the care team, works with clinic staff to develop goals for health and provide individualized Plans of Care for patients with chronic conditions.
- As part of each patient's assessment, discusses treatment goals and plans along with any barriers to the treatment plan.
- Provides information on new medications and explains referrals to care coordinators, social workers or other specialists.
- Participate in PCMH (Patient Centered Medical Home) related quality improvement initiatives at the clinic level.
- Other Duties as assigned.
QUALIFICATIONS
- Graduate of an accredited Registered Nursing Program.
- Current Ohio License for Registered Nursing.
- Graduate of an accredited Nursing Practitioner Program.
- Board Eligible or Board Certified as a Family Nurse Practitioner.
- Maintains continuing education units as required by the State of Ohio.
- Maintains requirements for a Nurse Practitioner with the Ohio Board of Nursing.
