Reports to: Director of Nursing
Job Classification: Hourly wage
Supervisory Duties: Ensures that all the needs are met for the clients, as well as supervising staff that is on the shift.
Minimum Qualifications:
- LPN license
- High School Diploma or equivalent
- Proficiency in infectious disease care
- Strong communication and interpersonal skills
- Willingness to learn and adapt to new technologies and procedures
- Experience in inpatient care, medical offices, or clinics is preferred

General Requirements:
- Licensed staff must be compassionate, respectful, patient, and possess a sincere desire to enhance the quality of life for the people whom they serve and their families.
- Capable of exercising good judgment and ensuring that Lakewood Landing will provide a safe, comfortable and secure living environment for the residents.
- Able to read and write English at an average adult level.
- Must be able to lift, push, and pull 50 lbs of weight without risk of injury.
- Able to work during half of the holidays recognized by Lakewood Landing as well as rotating weekends.
- Willing to undergo on-the-job training before being certified to work with residents including background check.
- Be flexible and willing to adapt to changing situations in a professional manner.
- Pass background check prior to providing care.
- Be neat, clean, and adhere to the tobacco-free policy.
- Must be efficient and well organized.
- LPNs are required to give a 3-week notice if resigning from the position.
Responsibilities include (but not limited to) the following:
- Provide companionship and supervise resident activities.
- Treat residents with complete respect and cooperation.
- Assist with any needs of the resident first.
- Check for Doctors orders that may come in. RN must be notified of any new orders that are received on your shift.
- Place pharmacy medication orders to be delivered to the facility.
- Obtain authorization from RN for any med changes.
- Communicate significant changes to the RN.
- Complete any med administration as ordered by the physician.
- Complete changes on MAR per physician’s orders.
- Prepare paperwork for any Dr. visits.
- Chart appropriately on clients, assist with weekly charting on clients.
- Assist with bathing, dressing and grooming as needed.
- Perform accucheck and insulin administration as needed.
- Assist with dining room duties.
- Pass meds and comply with regulations, policies and procedures.
- Attend some monthly staff meetings and any required training.
- Assist to serve meals when needed.
- Perform duties on a charting platform that pertain to you.
- Must be willing to attend on-the-job training sessions prior to being certified to work with clients.
- To be in attendance for scheduled shifts or have found a suitable replacement to work for you.
- Perform other responsibilities as assigned.

Pay: $28.00 - $32.00 per hour
