Job Summary: The
Director, Clinical Services - Homecare will have direct strategic impact to ensure high quality services are fully utilized within the existing service area. In addition, through building relationships within the community and with providers, he/she will work to grow the home care program within the business to capture lost referrals, increase volume and increase census.
Within clinical leadership, the Director interacts with hospitals across the System, Population Health as well as non-system providers to drive patient care initiatives for total patient care across the care continuum. Utilizing knowledge of industry regulations, standards, and trends, he/she will develop and evaluate goals and objectives for home health. He/she will direct and promote standards of clinical services to improve outcomes, process, and quality measures. Patient and caregiver satisfaction and trust must remain a high priority.
Oversight responsibilities include making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed and assuring the development, implementation and updates of the individualized plan of care.
The Director will collaborate with the internal Quality Assurance Performance Improvement (QAPI) coordinator on initiatives to ensure improvement plans are implemented and with key groups within the organization to ensure Homecare aligns with System initiatives. In addition, he/she will partner with the Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD) collaboratives to reduce patient readmissions. This leader will be an active participant on System committees such as Length of stay, Readmissions, Safety/Quality.

The Director will assume the role of the Executive Director/Administrator when the Executive Director/Administrator he/she is not able to be absent from the organization. The Homecare Executive Director/Administrator or Clinical Director is physically present or able to be contacted via telephone or other electronic means during all operating hours. The Director of Clinical Services is appointed by and reports to the governing body as an Alternate Administrator should the Administrator be unable to perform his/her duties.
Core Responsibilities and Essential Functions: People
- Evaluates the performance of Clinical Supervisors/Managers and implements training and development as indicated. Performs responsibilities of Clinical Supervisor/Manager in their absence.
- Ensures Clinical Supervisors/Manager manage productivity, employee engagement, department retention and turnover. Ultimately, responsible for the overall employee engagement, department retention, and turnover of home health.
- Perform yearly reviews on all direct reports and provide guidance, skill and techniques to Clinical Supervisors/Managers in evaluating the performance of clinicians.
- Reviews all recommendations for corrective action or discharge and assists in resolving complex employee relations problems.
- Review staffing levels compared to Admission volume to forecast staffing requirements, as appropriate.
- Ability to set expectations and hold accountable Customer Service.
- Ensures the home health department implements best practices to deliver service excellence.
- Performs home visits with clinical staff routinely.
- Ensures positive working relationship and effective communication with Wellstar departments and outside agencies.
- Represents Wellstar home health on appropriate committees throughout the health system.
- Ensures continued collaboration with all Wellstar departments to improve system and department outcomes. Clinical Outcomes Patient Satisfaction.
- Leads action plan implementation and completion with emphasis on Outcome Based Quality Improvement and Monitoring.
- Provides guidance and counseling to direct reports to assist them in continually improving all aspects of home health care services.
- Ensures proper maintenance of clinical records in compliance with local, state, and federal laws.
- Responsible for the evaluation of organizational performance via the performance improvement programs and assures the quality and safe delivery of home health services provided.
- Ensures compliance with all state and federal regulations and Joint Commission accreditation.
Financial
- Assist the Executive Director in the preparation and administration of the operational budget.
- Manage to expectation the operating and capital budgets for home health with emphasis on productivity.
- Interpret operational indicators to detect census changes and increases or decreases in volume, which could impact staffing levels, revenue or expenses.
- Oversee and manage the drop shipment of Med/Surg supplies ensuring patient and clinician satisfaction as well as continuing to look for opportunities to decrease supply costs.
Successful Growth
- Utilize pro-active staffing strategies to ensure acceptance of all appropriate referrals.
- Works with the Executive Director of Homecare, and Senior leadership to recommend and implement new programs or services.
- Works in conjunction with the Executive Director of Homecare regarding strategic planning and process improvement across service lines.
Required Minimum Education: Bachelor's Degree Required
Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated.
- Physical Therapist or Reg Nurse (Single State) or RN - Multi-state Compact.
- Basic Life Support or BLS - Instructor.
Additional License(s) and Certification(s):
A person who is a licensed physician, physical therapist, speech-language pathologist, occupational therapist, audiologist, social worker, or a registered nurse in the State of Georgia Required.
Required Minimum Experience:
Minimum 8 years 10 years of nursing or physical therapy experience Required and

When the Executive Director/Administrator is absent from the organization, the Director of Clinical Services will be designated to perform the duties of that position and will meet the following minimum experience: for individuals that begin employment with an HHA on or after January 13, 2017, a person who: (i) Is a licensed physician, a registered nurse, or holds an undergraduate degree; and (ii) Has experience in health administration, with at least 2 years of supervisory or administrative experience in home health care or a related health care program. 484.105 Required.
Required Minimum Skills:
Computer/data entry experience.
Ability to communicate with various members of the healthcare team.
Ability to use EXCEL, Word and have basic computer operational knowledge.
Epic and 3M experience preferred.
Demonstrated ability to lead, motivate, mentor, and direct professional personnel while responding to growth, change, and new programs.
Knowledge of business management, governmental regulations, OASIS and Joint Commission standards.
Must possess excellent written and verbal communication skills as well as proficiency in problem solving and time management.
Ability to implement processes. Knowledge of SHP and EPIC preferred.
