We’re looking for a full-time Patient Care Coordinator to support our home health division in Los Angeles.
This is an office position, working Monday through Friday, 8am to 5pm. No hybrid or remote options.
$40- $45 per hour
QI experience is strongly preferred.

What you will do:- Maintains schedule of all Home Health services.
- Assists with preparation of nursing call schedule.
- Receives updated orders from physicians.
- Reviews referrals and prepares clinical notes for interventions rendered.
- Participates in interdisciplinary team meetings and case conferences when available.
- Facilitates development and evaluation of the plan of care, under the supervision of the Registered Nurse Clinical Supervisor, and in partnership with the patient, the representative (if any), and the caregiver(s).
- Communicates with all physicians involved in the plan of care, and other health care practitioners (as appropriate) relating to the current plan of care.
- Facilitates counseling and education for the patient, family, and caregiver(s) as needed based on individualized needs, and under the direction of the Registered Nurse Clinical Supervisor.
- Performs nursing care and other field staff functions as necessary.
- Prepares and submits all case documentation, including progress notes, as appropriate, and payroll information in accordance with Agency policy.
- Acknowledges patient rights including privacy and confidentiality.
Who you are:- Graduate of an approved practical (vocational) nursing program.
- Licensed Vocational Nurse licensed by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
- At least two years of patient coordination experience or equivalent in-home health, hospital or medical group.
- Working knowledge of Medicare Conditions of Participation.
- Current CPR card.
- Core competency per skills checklist.
- Current health clearance.
