SUMMARY
The Admission Nurse coordinates admissions, assesses appropriateness of potential residents, provides case management, and discharge assistance planning from inpatient or community settings to the facility. The individual acts as a liaison to the community, acute care facilities, healthcare providers, and professionals seeking nursing home placement. Develop community partnerships with hospitals, healthcare providers, and professionals to enhance nursing home referrals and placement.
Market the facility to hospitals, healthcare providers, and professionals to encourage and develop current and future referrals.
Maintain ongoing communication and relationships with hospital discharge planners and social workers to enhance placement referrals.
Serve as a resource for residents, responsible parties, hospitals, healthcare providers, and professionals on issues related to nursing home care and services.
Coordinates discharge plans to ensure smooth transitions from hospitals, healthcare providers, and professionals to the facility.
Use assessment skills to determine potential residents' required level of care and communicate information to appropriate departments.
Communicate to the interdisciplinary team clinical and durable medical equipment needs for all new admissions and re-admissions.
Perform initial assessments of all individuals referred for nursing home placement to determine appropriateness of admission. Receive, monitor, and assess all potential admissions for nursing home placement.
QUALIFICATIONS
Required:
Graduate from an accredited school of Nursing as a Licensed Practical Nurse required.
Current Georgia License as a Practical Nurse required.
Ability to work independently with limited supervision.
Ability to travel to various hospitals, healthcare centers, and other locations to assess patients within a 50-mile radius of the facility.
Ability to communicate with other healthcare professionals.
Ability to work with various departments within a long-term care setting. Strong verbal and written skills.
Must have a valid driver’s license, reliable transportation, and proof of insurance.
Preferred:
3-5 years of experience preferably in a Long-Term Care setting.
1-2 years of marketing experience preferred. Prefer competency in bilingual language speaking and writing. Ability to create and maintain professional relationships.
Knowledge of Medicare, Medicaid, HMO, Managed Care, and private insurance payer sources reimbursement for skilled nursing facilities.
Knowledge of Medicare RUGS IV categories, Georgia Medicaid Case Mix Index, MDS 2.0, and MDS 3.0. Knowledge of ICD 9 Codes.
Knowledge of CMS, State of Georgia, and Joint Commission nursing home rules and regulations.
