JOB SUMMARY:
Responsible to the RN Clinical Manager and the day-to-day management of staff and the coordination of the care of each patient/family admitted to a Hospice of Marion County, Inc. Field Team.
ESSENTIAL JOB RESPONSIBILITIES:

- Acts as a liaison between all levels of patient/family care, internal customers and the professional community and assists the RN Clinical Manager (RNCM) with the creation/coordination of the patient’s plan of care and promotion of Interdisciplinary Team (IDT) communication/cohesiveness to help ensure a smooth transition and continuity of care throughout the 24 hour day.
- Responsible for the timely completion of annual competencies for nursing personnel (RNs, LPNs, CNAs) as assigned, which includes an evaluation of their understanding of the Hospice philosophy in accordance with agency policies and procedures.
- Provides guidance, support and supervision to team members while facilitating team functions and problem resolution of patient/family care issues.
QUALIFICATION REQUIREMENTS:

- Registered Nurse with a license issued by the Florida Board of Nursing. Bachelor Degree in Nursing preferred.
- Minimum of (2) two years clinical nursing experience; with at least (1) one year hospice experience; (1) one year leadership experience preferred.
- Current Basic life support (BLS) CPR Certification required.
