Your Role: The Case Manager advocates for the residents that participate in HCS Possibility Center, in all aspects of their residential life. This requires teamwork with all departments to ensure a high quality of life, respect, dignity, and protection of individual rights.
What We’re Looking For:
- Bachelor’s in Social Services, Special Education, or related field.
- Minimum of two or more years working with individuals with developmental disabilities.
- Proficient computer skills and ability to learn various web-based applications.
- Detail-oriented and effective communication skills to interact with all staff, departments, families, and outside entities.
- Lift residents weighing up to 35 pounds independently as needed; assist in lifts over 35 lbs.
- Valid driver’s license with no more than 5 points on license.
- Must submit to a criminal background check and be determined to be not disqualified by the Department of Human Services.
- Employee must submit to drug screening prior to employment, randomly, and for cause.
- Must agree to have name checked against the Central Registry of Offenders against Individuals with Developmental Disabilities and Child Abuse Record Information.
Your Day-to-Day:
- Provide atmosphere that maintains the safety, health, dignity and well-being of the resident.
- Encourage and increase independence of each resident through development of appropriate goals and training.
- Assist Managers with annual goals and resident assessments.
- Ensure all in-house trainings are completed for all staff.
- Assist with Medicaid renewals and guardianship paperwork.
- Attend all ISPs and ensure the final document is in the individual’s file.
- Monthly review of Therap (daily notes, sleep logs, and input information for Initial GERs).
- Check monthly reports after managers complete them. Check other monthly paperwork, including but not limited to data, activity sheets, fire drills.
- Cooperate with Hope Christian Services and the licensee and Department of Human Services staff in any inspection or investigation.
- Complete internal and State incident/accident reports as needed.
