Practice or research specialty/focus of the position: Adult Malignant Hematology
Position Summary: The Oncology Services Division of the Duke University Hospital Department of Pharmacy is looking for a highly motivated and skilled practitioner to join our team of inpatient oncology pharmacists in delivering exceptional care to adult malignant hematology patients. In addition to providing clinical pharmacy services, including rounding, limited order verification and clinical monitoring, responsibilities include:
- Partnering with physicians and other providers to provide clinical pharmacy services and to promote evidence-based, safe, effective drug therapy (M-F).
- Serving as the medication expert to nurses, physicians, and other healthcare staff by providing drug information and ensuring the optimum use of medications.
- Providing education to patients.
- Providing comprehensive supportive care and other medication therapy management.
- Partner with ambulatory colleagues to optimize patient transitions of care.
- Maintains outstanding skills as a clinical pharmacy practitioner and consistently demonstrates the value of pharmacy services.
- Educates medical, nursing, and pharmacy staff about the management, prescribing and monitoring of therapies used in patient care area(s) of responsibility.
- Serves as a resource to the Department of Pharmacy for area(s) of expertise.
- Provides educational lectures and/or participates in research (Duke, State/Local, National).
- Works with peers to achieve division and departmental goals.
- Takes responsibility for personal and professional growth.
- Maintains familiarity with all departmental programs and works as integral part of the pharmacy team.
Minimum Qualifications
Education: Doctor of Pharmacy Degree OR BS Pharmacy Degree
Experience: With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required. With Doctor of Pharmacy degree, no experience is required. 2+ years of practice experience desired. PGY1 or Pharmacy Practice Residency desired. PGY2 in oncology, Internal Medicine or Palliative Care or equivalent experience desired.
Degrees, Licensures, Certifications: Licensed Pharmacist or eligible to practice pharmacy in North Carolina. Hold or Achieve Board Certification in Oncology Pharmacy or Pharmacotherapy within one year of hire.
