The VNA Health Group is seeking a Full Time Registered Nurse Hospice Liaison to join our dedicated Hospice Team in Monmouth County. The position would be responsible for assessing patients for hospice needs and transition of service and coordinating total care for hospice patients and their families.
Responsibilities
- Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with co-workers.
- Assesses the needs of the patients and provides counseling to patients and their families and coordinates the transition to hospice to promote continuity of care.
- Establishes and maintains liaison with physicians, external and internal customers.
- Assists in managing and monitoring the quality and appropriateness of the services and programs.
- Conducts community outreach activities to improve community access to hospice care.
- Assists in developing and implementing a discharge program at Centrastate Medical Center and Monmouth Medical Center.
- Adheres to agency infection control and safety policies.
- Adheres to the organization’s policy in regards to absenteeism and appearance.
- Omission of specific duties does not exclude them from this position if the work is similar, related, or a logical assignment for this position.
Qualifications
- Bachelor’s degree in Nursing required.
- Excellent clinical skills and experience in hospice nursing and community health nursing.
- Excellent verbal and written communication skills.
- Excellent customer service, organizational, and interpersonal skills.
- Strong computer skills with proficiency in MS Office Suite.
Working Conditions/Physical Demand: Fast-paced health care environment with phone, scanner, email, fine motor hand movement, and computer use. Use of personal automobile with valid insurance and driver’s license.
