What You Should Know About the RN Care Manager Role:
- The RN Care Manager is a driving position and will require travel throughout Franklin County.
- This is a Full-Time position, Monday-Friday 8:00 AM - 4:30 PM with holiday rotation.
- We provide superior care and services to patients at their end-of-life journey. Only those with a heart for hospice will succeed.
The RN Care Manager's Essential Duties:
We provide superior care for patients and families in homes/facilities. The Care Manager plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. You will effectively interact with patients, families, and other interdisciplinary team members while maintaining professional nursing standards.
Qualifications:
- Active Registered Nurse license in the State of Ohio without any board actions.
- Two years of acute care nursing experience preferred.
- Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment (Eligible upon completion of 500 hours in the last 12 months of hospice/palliative experience).
- CPR Certified.
- Computer skills sufficient to properly document services and care.
- Ability to drive during daytime, nighttime, or inclement weather.
- Valid Driver's License with Safe Driving Record.
- State Minimum Automobile Insurance Coverage.
