Opportunity for a Family Medicine, Internal Medicine, or Geriatric Medicine Nurse Practitioner or Physician Assistant to join a post-acute care facility in Olympia, WA area.
Full-Time – 4 days per week
Schedule:
- Monday – Friday – Set own schedule
- Weekday call requirements only – 7am – 7pm – telephone only
Key Responsibilities:
- Perform primary care in a post-acute setting for short-term and long-term patients
- Manage chronic diseases, diagnose illnesses and medical conditions, and implement a plan of care
- Order and interpret lab results
- Prescribe and monitor medications
- Consult with physicians and coordinate patient care with other healthcare practitioners
- Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
- Build relationships with patients, patients’ family members, and facility staff to facilitate effective treatment delivery
- Promote healthy lifestyle changes
Requirements:
- New grads welcome to apply
- Primary care experience a plus, post-acute care experience a plus, but not required
- Active and unrestricted Washington NP or PA license required
- Active and unrestricted DEA required
Compensation and Benefits:
- The compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential
