Are you a nurse that loves working with children and ready for a change? Join our team as a case manager for the Technology Dependent Waiver and the Medically Complex Children's Waiver. Nurses perform advanced supervisory tasks for the home and community-based waiver for individuals served on the Technology Dependent Waiver and the Medically Complex Children's Waiver. Provide case management functions, exercise full professional discretion and technical judgment based on licensure in public health programs. Occasional travel is required for site visits. A licensed Registered Nurse is required for these positions. Preference may be given to applicants for the following:
- Pediatric experience
- Experience with medical technology
- Experience dealing with medically complex children and adults
- Spanish speaking
Note: as of April 8, 2024, DHHS employees will be working a hybrid schedule, which includes 2 office days per week.

EXAMPLE OF DUTIES
- Develops or modifies rules, policies, or standards, etc.
- Develops person-centered care plans.
- Provides medical case management and consultation in specialized areas of knowledge.
- Performs physical examinations and/or preventive health measures to assess and evaluate patient's clinical problems and health care needs.
- Provides direct and/or indirect patient care oriented toward improving or maintaining health and preventing illness.
- Provides clients with information concerning rights, options, benefits, services, goals and expectations.
- Assesses situation and applies appropriate interventions to prevent or resolve crisis situations.
- Ensures compliance with applicable federal and/or state laws, regulations, and/or agency rules, standards and guidelines, etc.
- Adapts program(s) to meet specific needs or concerns.
- Maintains and updates patient charts, records, and information.
SUPPLEMENTAL INFORMATION
Licensure as a registered nurse (RN) in the state of Utah. Effective July 1, 2021, State of Utah Employees must either reside in Utah or be in the process of relocating to within 30 days of their start date. This position may currently be a hybrid of both in-office and remote work days. Please note, a position's eligibility for remote work is established by agency management and is subject to change at their discretion, at any time and for any reason. Risks which require the use of special safety precautions and/or equipment, e.g., working around operating machines, working with contagious diseases or hazardous chemicals, etc. Work requires physical exertion. May require the ability to stand; walk over rough surfaces; bend, crouch, stoop, stretch, reach, lift moderately heavy items (up to 50 lbs.) in a recurring manner and/or for long periods of time.
