We are a small, 32 bed Continuing Care Retirement Community (CCRC) offering a full spectrum of care from retirement living, personal care, licensed healthcare, rehabilitation services, respite to memory care services.
Job Title: LPN Team Leader
Status: Full Time Days working 6:30am-3pm, including every other weekend and holidays
Salary Range: Experience based pay starting at $30/hr PLUS Shift Differentials including $2 for Evenings, $1.50 for Nights and an additional $1 for Weekends!

As a LPN Team Leader, supporting our mission will include the following responsibilities:
- Observe and report changes in resident condition to the Nurse Supervisor or physician/physician extender
- Communicate with Supervisor, ADON, DON, NHA, care providers and family/POA as needed
- Uses evidenced-based practices to ensure care delivery is provided to residents
- Assist in the management of personnel through on-going performance evaluation, Care Chats, coaching, counseling, education, and Corrective Action as needed
- Complete all required documentation accurately and timely
- Participate in resident TOC/Care Conferences and care plan review as needed
- Collaborate with Cedarwood staff to ensure optimal care for residents
- Comply with Infection Control practices
- Maintain hand-held communication equipment is maintained by all direct-caregivers for effective call bell management & communication with team members
What do I need for this role with Frederick Living?
- Licensed Practical Nurse in the Commonwealth of Pennsylvania
- Prior experience in geriatric nursing preferred
- High school diploma or equivalent
- BLS Healthcare Provider
- Basic computer skills
