Job Purpose
Reporting to the Director of Behavioral Health, the Peer Recovery Specialist is responsible for providing support to individuals with substance use and/or mental health concerns. This role coordinates with FHCHC clinicians and community organizations to identify and address the needs of clients, provide intervention and support services, and assist clients in achieving therapeutic goals. The ideal candidate has a deep understanding of recovery and harm reduction processes, is highly organized, and has excellent communication and interpersonal skills.
Duties and Responsibilities
- Develop and implement individualized recovery plans for clients, including goals and objectives.
- Practice a strengths-based approach to recovery and wellness.
- Engage peers in collaborative and caring relationships.
- Document encounters in the electronic health record, as appropriate.
- Monitor client progress and provide ongoing support and guidance.
- Perform standardized screening instruments with clients, including assessment of social determinants of health.
- Connect clients to appropriate community resources and services.
- Provide crisis intervention and support services.
- Provide education and resources to clients and families.
- Facilitate weekly recovery groups.
- Maintain accurate records and documentation.
- Prepare and submit reports to funding agencies, as needed.
- Coordinate with other professionals to ensure continuity of care.
Qualifications
- Lived experience with substance use, mental health concerns, homelessness, and/or sex work.
- Recovery Support Specialist training and certification is preferred.
- At least one year experience working in a recovery-oriented setting.
- Demonstrated knowledge of substance use, harm reduction, and mental health services.
- Excellent communication and interpersonal skills.
- Able to work independently and as part of a team.
- Proficient in Microsoft Office.
