POSITION SUMMARY:
The Lead Registered Nurse (Lead RN), under the direction of the Director of Nursing and the clinical care team, serves as an integral member of the nursing and CCI health center leadership teams. As a member of the health center leadership triad, the Lead RN participates in administrative and organizational leadership activities to promote efficiency and optimal quality of care. They are responsible for assisting and supporting the CCI team in providing high quality, direct, evidence-based patient care and care managing high risk patients with chronic illnesses, to promote health education, condition management and self-advocacy to ensure improved patient and financial outcomes.
As KEY FUNCTIONS AND RESPONSIBILITIES:
- Serves as a lead for the health center through modeling and performing routine nursing care, care coordination and management duties to optimize patient care, upholding high quality and safety standards for CCI’s population of patients from infancy to geriatrics, which may include:
- Developing a working rapport with patients.
- Leading and participating in clinical huddles, pre-visit planning and identifying complex care patients.
- Conducting patient appointments in collaboration with clinicians - collecting patient health history, assessing, screening, triaging, analyzing, and identifying a patient’s healthcare needs and care gaps.
- Providing appropriate medical advice and discharge instructions to patients with the support of evidence-based resources for pediatric, adult, and obstetric populations.
- Reviewing and reconciling medical and vaccine records - completing required forms and paperwork needed by patients, schools, and other healthcare entities.
- Reconciling medication history and counseling patients on the use, benefits, risks, and potential side and adverse effects of any medications prescribed and administered.
- Coordinating clinical care, transitions of care and supporting the referral process.
- Counseling and education on chronic disease management, primary prevention activities, abnormal labs and diagnostic imaging and working with care teams to determine appropriate follow-up screening and notification to local and state agencies as indicated.
- Executes treatment plan as directed by the Director of Nursing, and/or clinician and evaluates results including providing point of care testing, vaccination and medication administration, refilling patient prescriptions and obtaining prior authorizations.
- Providing verbal and written information in the patient’s preferred language by utilizing in-person or telephonic interpreters.
- Maintaining a panel of care management patients through developing care plans, setting patient-centered goals and continually assessing patient’s self-advocacy, self-management and barriers to care.
- Collaborating with population health and leading monthly care management huddles, coordinating patient care with community health workers.
- Serves as lead for the nursing department at their health center and is an integral part of the leadership triad, which may include:
- Developing, enforcing and maintaining policies, procedures, workflows and clinical guidelines.
- Collaborating with Health Center leadership including the Health Center Manager and Health Center Medical Director, identifying concerns requiring resolution.
- Engages in health center and CCI Quality Assurance and Quality Improvement measurement, analysis and performance improvement activities to meet CCI quality goals.
- Supervising health center nursing staff – e.g., clinical nurses - including assigning duties, hiring, on-boarding, training, ongoing education, and corrective action.
- Collaborating with health center staff to oversee inventory and ordering of supplies, pharmaceuticals, and durable medical equipment.
- Plans, coordinates, and/or attends assigned meetings, health fairs, events, and trainings.
- Consistently seeks avenues for self-education to broaden knowledge base and maintain compliance with evidence-based practice.
- Uses the Electronic Medical Record to maintain patient medical records, utilizing standard language and abbreviations and following accepted medical standards for charting.
- Provides quality customer service and communicates in a HIPAA compliant, culturally sensitive, and trauma-informed manner.
- Works within the policies and procedures of CCI Health Services Inc.
- Performs other duties determined as necessary by the Director of Nursing and other CCI leadership.
MINIMUM QUALIFICATIONS:
- Active RN license issued by the State of Maryland Board of Nursing or any state under the Nurse License Compact.
- Minimum of three (3) years of nursing experience, with progressive leadership experience a plus.
- Current BLS/CPR certification required.
- Experience with documenting in an Electronic Medical Record (EMR) – specific knowledge of eClinicalWorks is a plus.
- Prior experience in a lead or supervisory capacity a plus.
- Written and oral fluency in English.
