About the Program and Position:
Our LPNs specifically provide medical care required for all the youthin foster care we serve. We are looking for LPNs who are passionateabout helping this vulnerable population and able to support a busymedical office. The role is crucial to the health & well-being ofour children and families who we serve.
Under the direction of an RN you will utilize your skills to…
Assist with initial/follow up medical consultations
Perform vitals and point of care testing (ie. urine pregnancytest, strep test, flu test, COVID test, glucose test)
Review immunization records, draw up and administer necessary immunizations
Update City Immunization Registry
Administer medications
Act as English-Spanish translator for medical provider
Care Coordination with both onsite and community medical/mentalhealth/specialty care providers
Work collaboratively with the Medical Provider, Case Planners,Clinicians, Behavioral Support Specialists, and Fair FutureCoaches to coordinate appropriate services for each youth
Responsible for blood draws, performing EKG, assessing youths fortransfer clearances, and performing initial 24 hour medicalscreenings upon intake
Responsible for all documentation with EMR (Netsmart/MEDENT)
Schedule: Exempt
Regular hours Monday - Friday 9:00am-5:00pm
Remote on-call may be required which may include weekends/holidays
Must be flexible to meet program needs
Experience and Skills:
NYS Licensed Practical Nurse
Experience working with youth
Strong communication and organizational skills
Ability to work effectively in a team context
Demonstrated ability to perform duties with minimal supervisionand to exercise greater judgment
Physical Requirements:
Word processing: Including responding to emails, composingletters, completing computerized documentation, be familiar withexcel and electronic medical record system etc.: This involvessitting at a computer for several hours
Completing paperwork -Such as copying, scanning and faxing documents
Must be able to lift up to 50 pounds
160 hours of vacation time; 12 Sick Days per year
