Medical Respite Personal Care Specialist, Northampton
CSO is looking for a Personal Care Specialist for our Medical Respite team in Northampton.
This program is a 10-bed Medical Respite, located centrally to the CSO service area in Northampton, MA. Medical Respite team members will support guests in healing, and ultimately transitioning to appropriate housing.
This position coordinates and monitors theassessment, treatment planning process and follow-up for each guest and serves as an advocateand liaison for guest benefits.
As a Medical Respite Personal Care Specialist
You Will:
- Complete new guest assessment to determine eligibility and needsfor social support programs
- Assist guests in the development of individual plans to movetoward self-sufficiency and to retain/secure permanent housing. Evaluate, facilitate and monitor guest'sprogress towards established goals and objectives.
- Support ADLs (Activities of Daily Living) such as: mobility, rangeof motion exercises, assistance with medications, bathing or grooming,dressing, eating, and toileting., and ADLs (instrumental activities of dailyliving) such as: laundry, shopping, housekeeping, meal prep, attending medicalappointments, and maintaining equipment
- Provide resource/assistance for guests in: budgeting, job training, employment search,locating permanent housing, managing medical issues and addressingtransportation and other needs
- Advocate for guests with other service providers, including federal,state and city agencies and other non-profits; provide referrals to other appropriateservice providers as needed (e.g. Social Security, Medicaid, Medicare)
- May provide transportationservice to guests for doctor appointments, job interviews, social security,veterans' association, and apartment/landlord meetings
Your Background:
- High School diploma or GED
- Twoyears' experience working as a CNA or Medical Assistant OR two years' experience in the Human Services Field suchas working with low income and homeless populations, chemical dependency,and/or mental health programs
- Validdriver's license and reliable transportation
