About This Hospice Patient Care Manager Position
The Hospice Patient Care Manager is responsible for the overall delivery of patient care services by Inter-Disciplinary Group (IDG) and provides direction and supervision to all IDG staff members.
The Hospice Patient Care Manager maintains close working relationships with staff to ensure patient care service needs are met.
This position will be based at Hope Health’s inpatient hospice center, the Hulitar Center, in Providence, RI.
This is a full-time position that is eligible for benefits. The hours will be 8:00am-4:30pm with every third weekend rotation, the hours may vary depending on the need of the IPU.
Duties / Responsibilities:
Clinical Services
- The Clinical Manager develops, administers, and oversees all aspects of the team and ensures the provision of high-quality services to patients, families, caregivers and outreach to the community.
- Monitor and oversee the efficient and effective cost human, fiscal, and material resources.
- Ensures nursing care and overall provision of patient care by Team IDG members is coordinated, safe, competent, and effective.
- Build and maintain relationships and communication with HPC-RI transition team to support customer relationships.
- Ensures the day-to-day activities and services delivered by interdisciplinary team in conjunction with managers and staff.
- Ensures and guides appropriate orientation of team staff and identifies opportunities for staff to develop new competencies, along with remaining current on required competencies. Assists in the development of educational programs for staff.
- Ensures effective communication of patient and family needs and ensures efficient coordination and provision of patient care 24/7.
- Responsible for identifying communication issues and facilitate resolution of the same.
- Represents the organization in the community.
- Oversees daily staff assignments to clinical staff. Monitor staff performance to ensure achievement of established goals.
- Works collaboratively with Finance and IDG members to resolve reimbursement issues.
- Provides guidance, mentoring, support and supervision. Acts as clinical resource for nurses and all other IDG members.
- Hires, supervises, monitors and evaluates all members of IDG.
- Responsible for performance reviews, clinical competencies, supervisory visit and all other performance related activities.
- Builds partnership and resources throughout the organization. Oversee effective interdisciplinary team care planning meetings and other operational meetings.
Performance Improvement/Regulatory
- Embraces and uses Performance Improvement principles and behavior. Participates in and leads quality improvement activities which support the organization’s mission, philosophy, customer service and process improvement goals. Develops and monitors key performance indicators as necessary.
- Collaborates with Quality and other managers in formulating, revising, implementing and evaluating organizational policies, procedures, goals, and objectives. Ensures nursing care is compliant with all regulations, standards, laws, and practice acts.
- Assists in budget development and accountable for ensuring adherence to budget guidelines and objectives. Reviews invoices as appropriate. Maintains appropriate resource management (human, fiscal, material).
- Ensures clinical documentation meets compliance with regulatory, quality, and reimbursement requirements. Ensures care plans are current, patient centered and meets patient needs 24/7.
- Reviews team records to ensure compliance. Responsible for development of action plans for improvement in identified areas.
- Actively participates in HPC-RI Leadership Team and other clinical and non-clinical projects/teams. Demonstrates collaboration with peers and staff in identifying ways to better provide patient care and staff education and support.
- Identifies potential risk management concerns and communicates information to IPU Director.
- Establishes and maintains positive relationships with customers. Handles concerns/complaints in a timely manner.
- Drives Passion for Excellence initiatives ensuring the implementation and achievement of defined goals.
- May act as Administrator on Call (AOC) on a rotating basis.
- Understands and supports efforts to create ease of movement within the Hope Health system with particular attention to be paid to GIP level of care and GIP criteria.
- Demonstrates accuracy, precision and execution of medication administration, documentation and general oversight.
- Demonstrated proficiency with medication administration, documentation, and general oversight of reporting.
- Understands and supports efforts to maintain census and remove obstacles to patient movement within the Hope Health system.
Program Development
- Identifies need for new programs or services and makes recommendations for improvement in operational performance. Collaborates with others to implement new programs and services to enhance patient care and staff education and guidance, including community outreach and collaborative.
- Serves as liaison between HPC-RI and other health and social service agencies and in a variety of settings.
- Pursues professional development opportunities and remains current on trends and development within the industry.
- Collaborates with clinical and non-clinical managers and staff and departments in developing and implementing innovative programs to better meet patient, family needs, and community needs to achieve organizational strategic goals.
- Responsible for budgetary requirements, Passion for Excellence, Employee Engagement, and performance review processes.
- Consistently apply the HPC-RI values in all aspects of work, complying with established HPC-RI mission, policies, procedures and objectives in quality assurance, safety, environment, infection control and HIPAA compliance.
- Drives Passion for Excellence initiatives ensuring the implementation and achievement of defined goals.
Education / Experience:
- Three to five (5) years nursing experience with in-depth clinical knowledge to include a minimum of two years nursing experience in a Home Care/Hospice setting.
- Evidence of applied knowledge in all state and federal insurance standards, regulations and accreditation standards.
- Hospital care and/or hospice experience preferred.
- Direct clinical supervisory experience required. Progressive experience with process improvement teams desired.
- The ability to achieve HPNA certification within one (1) year of employment.
- Preferred experience with process improvement teams desired.
- Demonstrated acceptable patient care and “high tech” skills.
- Currently licensed as an RN through the State Board of Nursing in Rhode Island.
- Ability to prove math competency skills including dosage and calculation.
