Community Memorial Hospital is a 12-bed, level 4 critical access hospital located in Sumner, IA, and serves the Bremer County area. The modern 12-bed inpatient area at Community Memorial Hospital offers a comfortable, quiet, and healing environment, staffed by professional caregivers who provide personal and attentive care. The inpatient area is designed to meet a variety of needs – including pediatric, adolescent, adult, and hospice care. CMH is committed to meeting and exceeding the high standards for safety and quality of care that you and your family expect and deserve. The emergency department at Community Memorial Hospital features life-saving technology and is staffed by physicians and providers and experienced nurses with trauma training.
- Established team of 4 physicians and 2 Advanced Practitioners
- Opportunity to practice outpatient practice serving Sumner and surrounding communities.
- Cohesive group of colleagues and supportive staff allowing for autonomy while building the practice according to your preferences
- Experienced staff, both clinical and non-clinical, who know the patients and the community.
- Full-time position, including dedicated administrative time, with flexible scheduling to ensure sufficient patient access
- Fully integrated system with clinic and hospital on EPIC EMR to improve care coordination

Responsibilities
- Ability to care for all ages in an outpatient clinic setting
- Family practice with emergency department coverage opportunity available
- Outpatient call is covered by provider on-call for emergency department of the hospital
- General family medicine procedures such as joint injections, skin excisions, and lacerations
- Clinic located within the hospital- X-ray (imaging services), digital mammography, Lab, and other outpatient services available
- In house and visiting specialties include Ortho, ENT, Podiatry, Cardiology, Urology, Nephrology, Oncology, and General Surgery
- Patient-focused, friendly, and willing to collaborate in a team environment
- Provide the best outcome for every patient every time
- Ability to work as a team member to improve the health of the people and communities we serve
Qualifications
- Nurse Practitioner or Physician Assistant
- Board Certified
- Experience preferred but not required.
- Hold an unrestricted medical license in the state of Iowa
- Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
- Eligible to work in the United States

Community Details
Sumner is the best-kept secret in Northeast Iowa. Surrounded by rich farmland with big city amenities but come with a small-town feel. It is a short drive to bigger communities including an hour’s drive of five colleges and university. Populations of 2,021 and was established in 1870. There are many historic buildings in Sumner, including the old bank/clock tower, and the old school building, which was built in 1901 and is currently a museum. Sumner has recently built a new library, recreation center, and a park for residents. Sumner... a place to call home, a place to work, a place to shop and relax.
