Endocrinology Job Opening in State College, PA Area is surrounded by state parksLocated 140 miles from Pittsburgh; 170 miles from Baltimore, MD Position Details: - Full-time, permanent- Looking to employ an Endocrinology physician who can become the leader of the Division.- Rate of transition to the leadership position will depend on the experience of the successful candidate. Mentored transition over several years is an option.- The group currently has 2 Endocrinologists, 1 NP and 2 PAs. Looking to add another PA/NP soon. - Diabetic patients are seen with 4 co-located Diabetes Educators.- The catchment area of the practice is large and will see patients with all usual and some unusual endocrine problems. Standard endocrinology testing is available, including Insulin Tolerance Testing.- Diabetes care focuses on population health. This part of the program is led by one of the NPs. The vision is to have a unified approach to diabetes care across the primary care endocrinology hospitalist diabetes education continuum. - Applicants must be Endocrinology BE/BC. - Several years experience in the practice of endocrinology is preferred.- The group offers a competitive compensation package- M-F work schedule, 8:30am-5:00pm, providers determine how they would like to schedule. - All diabetes patients are scheduled for a new appointment with CRNP, RD and CDE. Endo patients assigned on a 2:1 bases favoring the new Endocrinologist. Decisions are made collaboratively with the broad mission and philosophy of practice. Currently the Division of Endo provides almost no inpatient care. All potential inpatient consults are screened by phone (less than 1 per week) and the vast majority of issues are addressed by providing advice on the phone with outpatient follow up. There is no direct inpatient care responsibility. The Division also does not provide direct phone access to patients. All patients have primary care providers and are encouraged to call PCP first. Growth and leadership potential exist across the spectrum of activities. Leadership help / assumption of leadership in any of the areas of activity would be welcome.- Inpatient responsibilities for the endocrinologists are limited, but will likely increase as the health system grows. Hands-on inpatient diabetes care is provided by a team of pharmacists as requested by consultation from hospitalists. The Division of Endocrinology provides oversight for this program. Inpatient care is a major focus, and Mount Nittany is one of only a few acute care hospitals in Pennsylvania with Joint Commission diabetes inpatient care accreditation.- Gestational diabetes care is another major focus, with guidelines in use here often ahead of national guidelines.- In addition, the Diabetes Foot Clinic is one of the premier Diabetes Foot Clinics in the country, with a strong record of original research as well as patient care. The Division of Endocrinology collaborates closely with this program. Components of the Program: - Guideline based processes for transition from outpatient to elective inpatient care, typically surgery.- Clearly defined processes for transition from inpatient to outpatient care, assuring 24/7 support for patients newly diagnosed with diabetes, newly taking insulin, etc - Guideline based and seamless transitions or co-management between primary care and specialty care.- Group and individual diabetes education at all primary care sites and in the community.- Motivational Interviewing training for practitioners. Key to the program is the belief that, in addressing a chronic disease, a major if not primary responsibility of a specialty practice in a large multidisciplinary physician group is to support care of that chronic disease in primary care and across the health system more generally. In that regard all diabetes population health management guidelines and processes are developed as a collaboration between specialty care and primary care, or other stakeholders. Examples of practices implemented in primary care and co-developed with specialty care include:- Foot exam reminders and standard workflow.- Eye exam reminders and standard documentation of data obtained from eye care providers.- Standards and workflows for teaching glucose monitoring and insulin administration.- Workflow for examining SMBG data in primary care.
