A large Boston area health system is seeking compassionate, experienced APPs for its new "Hospital at Home" program, going live in August 2023. Promising to be a game changer, patients admitted to the hospitalist service will receive care from the comfort of their own homes!
Responsibilities:
- APP will see about 5-6 patients per day. Patients will be seen by an APP every other day.
- Hospital/Tele Command center Physicians will be available for video visits and back-up.
- Procedures may include Foley cath, IV line placement, drawing blood, etc., and managing medication with a virtual RN from the command center.
- Utilize EPIC EMR.
Must be comfortable working autonomously and interfacing with patients and their families in their homes. Outstanding communication and problem-solving skills are required. Ability to work well within a team.

Minimum Requirements:
- 3 years of relevant experience. Acute Care/Home Health settings preferred.
- Strong Communication Skills including problem solving, conflict resolution, and planning.
- Empathetic, patient-centered approach.
- Flexibility/Adaptability to dynamic/changing situations and environments.
