Position Summary
At Stony Brook Medicine, the Assistant Director of Care Management is a valuable member of our team, who provides empathic care to our patients, families, and visitors, with the highest degree of integrity and respect. Care Managers at Stony Brook apply the principles and practices of their profession to provide evidence-based clinical care, supervisory oversight, and project management as well as assessment and planning. The Assistant Director of Care Manager reports directly to the Director of Care Management assisting with oversight and supervision of the care management staff, as well as multiple projects focusing on process improvement and quality in regards to Care Management and Social Work at Stony Brook University Hospital. They maintain compliance with regulatory standards within the scope of Care Management including outcomes management, utilization and appeal management, reporting/data analysis to leadership and staff.
Duties of an Assistant Director of Care Management may include the following but are not limited to:
- Responsible for addressing Utilization management, denials and appeal, discharge and transition planning activities, quality reviews in order to optimize patient outcomes within appropriate time frame with upper and lower level leadership.
- Coordinates the design, development, implementation and monitoring of the organizations case management and utilization review functions.
- Manages daily operations, which includes oversight of all Case Management leadership and staff functions regarding care management and utilization management activities.
- Manages human resources utilization, promotes employee satisfaction, supports staff development and utilizes the progressive discipline process when appropriate.
- Maintains goals to achieve clinical, financial, and utilization goals through effective management, communication and role modeling.
- Internal resource on issues related to the appropriate utilization of resources, coordination of care across the continuum, utilization review, discharge and transition planning and performance of leadership.
- Performs chart review and oversight of documentation compliance.
- Staff Supervision, evaluations and performance program review.
- Project management and assignment follow up. Involved in complex discharge cases.
- Maintain human resource management, leadership, quality and operational management objectives.
- Process improvement and quality improvement oversight. Education and presentations to the department, other hospitals, meetings and committees as assigned.
- Form and policy creation from the Director.
- Overall involvement in process improvement for multiple areas such as CTO and CIN.
- Attendance at leadership meetings with the Director or acting as when indicated.
- Assist Director in program development and unit performance improvement activities. Demonstrates core values of Stony Brook Medicine. Is a role model to all employees which is to provide high-quality, cost-effective healthcare for the people in the communities served.
Qualifications
Required Qualifications:
- Possession of license to practice as a registered nurse in New York State.
- Minimum 3 years RN supervisory experience.
- Minimum five years Care Management experience.
- Hospital tertiary experience.
- Weekend coverage for CTO.
- Ability to return to the hospital for crisis intervention.
- Mentoring and training other leadership and staff.
- Demonstrates strong leadership skills.
- Experience in multiple patient settings, i.e. medical, surgical, pediatric etc.
- Demonstrates strong skills in communication, initiation, independent work and project follow through.
- Knowledge of regulatory requirements such as CMS, EMTALA, etc.
- Intermediate in all MS Office applications.
- Working knowledge of InterQual and/or MCG.
- Knowledge of length of stay, readmission, inpatient vs. observation.
Preferred Qualifications:
- ACM, CMAC and/or CCM certification.
- Bilingual.
- Experience in a large teaching hospital.
- Experience with homecare/home IV infusion, SNF, AIR.

Anticipated Pay Range: The starting salary range (or hiring range) for this position is - $119000 - $149000 / year.
The above salary range represents SBUH’s good faith and reasonable estimate of the range of possible compensation at the time of posting. The specific salary offer will be based on the candidate’s validated years of comparable experience.
