Essential Functions:
- Provide comprehensive medical care and management for geriatric patients, including diagnosis, treatment, and preventive care interventions tailored to the unique needs of older adults.
- Conduct thorough geriatric assessments to evaluate physical, cognitive, functional, and other aspects of health, collaborating with interdisciplinary teams to develop personalized care plans.
- Manage complex medical conditions commonly associated with aging, while prioritizing patient safety, comfort, and quality of life.
- Coordinate with specialists, allied health professionals, and community resources to ensure seamless continuity of care and support services for geriatric patients.
- Educate patients, families, and caregivers on health issues, disease management strategies, medication management, and preventive health measures, empowering them to actively participate in their healthcare journey and make informed decisions.
Requirements:
- Board certification or eligibility in Internal Medicine.
- Valid medical license to practice in New Jersey.
- 2+ years of relevant experience.
- Strong clinical skills and experience in providing primary care to adult patients.
- Familiarity with geriatric medicine and a passion for working with older adults.
- Handle admissions and quarterly reviews.
- Excellent communication and interpersonal skills, with the ability to establish rapport with patients, families, and interdisciplinary healthcare teams.
- Ability to comply with regulatory requirements, documentation standards, and ethical principles in healthcare delivery.
- Local travel required.
