POSITION SUMMARY
An inpatient medicine-based practice in which the Nurse Practitioner or Physician Assistant completes comprehensive history and physical exam, and provides diagnostic and therapeutic care to adult inpatients with a variety of illnesses. Responsible for spectrum of patient transitional care (admitting, transferring, discharging). Provides patient-centered care within a collegial, multi-disciplinary team-based practice. The APP will practice in autonomous collaboration with a Hospital Medicine physician. Will report to the Hospitalist Medical Director.
BASIC KNOWLEDGE
A board certified or board eligible Nurse Practitioner or Physician Assistant.
EXPERIENCE
Minimum of 2 years of experience in direct patient care required as a Nurse Practitioner or Physician Assistant.

Acts independently within the customary scope of nursing or physician assistant practice and performs medical functions within a designated scope of practice.

ROLES AND RESPONSIBILITIES
- Provide patient-centered, standard of care to patients requiring admission, subsequent care, and discharge from the Hospital Medicine service.
- To practice independently to the limit of medical licensure, within the Elizabeth Community Hospital bylaws, with support and collaboration of a designated Hospital Medicine physician.
- Effort includes initiation of, review and interpretation of diagnostic testing, implementation of appropriate treatment plan, clinical follow up, collaboration with ED and specialty consultants, collaboration with interdisciplinary inpatient team (nursing, care management, rehab services, respiratory therapy, etc), managing effective and safe transitions out of the hospital in collaboration with nursing facilities and outpatient providers.
- ACLS certification is a requirement of the position.
- Attend monthly staff meeting either in person or remotely.
PHYSICIAN COLLABORATION
- Every shift a collaborating physician will be clearly designated.
- Inpatient admissions and discharges to be done in collaboration with a physician.
- For admissions, APP is expected to complete admission orders and H&P, staff patient encounter with collaborating physician and incorporate that input into the orders and note.
- The physician is responsible for attesting to and cosigning the H&P and Discharge Summaries.
DUTIES
- Will see patients on a scheduled basis as established by the practice. Will assess the patient’s health status which will include history taking and a physical examination addressing the chief complaint.
- Will order appropriate tests and procedures as indicated by the diagnosis.
- Will review laboratory and other diagnostic procedure data and according to established protocols, will assess the patient’s medical needs.
- Will establish a plan of care related to the patient’s identified problem including medication and diet. Will recommend appropriate therapeutic interventions.
- Will arrange physician referrals when appropriate, coordinating care with appropriate support agencies.
- Will chart patient information in the medical record in a timely, accurate manner in accordance with office policies, including patient appointments, telephone calls, and prescription refills.
- Will identify patients with extremely complex health problems and consult appropriately with collaborative physicians regarding proposed treatment.
- Will counsel and teach patients on a variety of health care matters such as self-care appropriate to the patient’s medical problem, preventive interventions, wellness programs.
- Will be actively involved in creating education materials and promoting educational programs appropriate for the patient population.
- Will develop a professional relationship and support all staff in order to enhance patient care and efficient clinical operations, acting as a role-model for nursing support staff. Shall abide by the policies and procedures of the practice and of Elizabeth Community Hospital.
- Will observe confidentiality policies established by the practice and develop a relationship with patients displaying professionalism, sensitivity, empathy and respect.
- Employees will perform their job in a safe manner as defined in any and all applicable Elizabeth Community Hospital policies specific to the job including but not limited to policies addressing: Universal precautions, Use of PPE, Safe lifting/ergonomics, Handling of hazardous materials.
