Our Care Partners provide a holistic approach to meeting resident care needs, assisting residents with daily living needs including meal service, light housekeeping, laundry, and programming in addition to providing activities of daily living. They provide encouragement and guide residents to maintain or reach their highest functional level, recognizing the individual’s needs and providing a home-like atmosphere within a strong team support.
Shift: 7pm to 7am
Duties/Responsibilities
- Provide redirection, reassurance, and emotional and psychological support to residents and family members in a caring, home atmosphere.
- Initiate and lead activities for residents. Facilitate socialization and participate in planned activities to the extent that the resident is able and willing.
- Ensure that residents have a pleasant dining experience. Allow the resident to smell the aroma of food, making the experience as similar to home-like as possible.
- Allow resident to participate in cooking activities by providing a safe environment for resident to experience and reminisce the cooking experience.
- Ensure that resident’s food is accessible, self-help devices are available, and food is appropriately arranged for visually impaired residents.
- Record resident’s food intake and/or output as directed and report changes in eating habits to the charge nurse.
- Assist with food set-up and feeding of patients. If tray service is utilized, only on an as-needed basis, ensure timely removal.
- Ensure dignity and grace in assisting residents with personal care. Ensure that bathing, dressing, grooming, mobility, toileting, oral hygiene and incontinence care are performed in accordance to residents care plan. Allow them the time to play an active role in choices and performance.
- Keep incontinent residents as clean and dry as possible at all times. Maintain this through regular monitoring and reminders. Change bed linens and clothing as often as necessary.
- Observe and report any loss of integrity to the skin.
- Assist licensed nurses, as directed, in collection of specimens, i.e., stools, urine, sputum, etc.
- Check and report bowel movements and character of stools, as directed.
- Provide a nurturing touch through light hand and back massage. Base this on resident’s desire to accept touch.
- Perform walking rounds at start and end of shifts to ensure safety and communication of needs.
- Complete light housekeeping duties including meal service, washing clothes, daily housekeeping, and trash removal. Keep areas clean and uncluttered.
- Encourage and engage resident involvement in meaningful activities whether it is a scheduled program or a spontaneous activity based on resident need or behavior at that moment.
- Safety for residents and neighborhoods must be maintained. Routinely complete safety rounds.
- Assist resident in preparing for scheduled appointments/planned activities, e.g., x-rays, lab work, physician visits, or religious services.
- Transport residents to and from designated activities by appropriate means.
- Assist in or perform in activities and nursing procedures, as instructed by Charge Nurse or Nursing Supervisor.
Qualifications
- Graduate of an accredited technical school for Certified Nursing Assistants.
- Minimum one year in the field of Long-Term Care preferred.
- Must have completed the Positive Approach to Care training.
- Must have good organizational skills.
- Must be a detail-oriented person and able to perform duties with great accuracy on a daily basis.
- Must have excellent personal skills, demonstrating the ability to conduct oneself in a non-controversial style; a style that inspires the respect of others and promotes a sense of trust in the individual’s competence.
- Must have good communication skills, speaking, writing, and listening.
